Abstract:

Introduction: Duodenum is the most common site for diverticular disease of small intestine. Most of duodenal diverticula are asymptomatic or have non-specific upper gastrointestinal symptoms. The complications of duodenal diverticula depend on the location and include acute diverticulitis, perforation, hemorrhage, and obstruction of the biliary or pancreatic ducts. Duodenal diverticulum can be diagnosed with upper gastrointestinal endoscopy or radiographic series.Case Presentation: In this article, we present a case report of a large duodenal diverticulum, treated with laparoscopic SURGICAL resection. After an accurate diagnosis; treatment modality should be determined by its localization, type and size. Laparotomy is the gold standard SURGICAL treatment of complicated duodenal diverticular disease. Conclusions: For laterally localized and protruded duodenal diverticula, laparoscopic resection is a feasible and safe method. The role of laparoscopy in SURGICAL treatment of complicated diverticula, located in other parts of the duodenum, is unclear because of lack of experience.

Abstract:

Background and Objectives: Surgery is the main treatment of pilonidal disease. Management of post EXCISIONal cutaneous and subcutaneous defect is an important problem. Various techniques have been introduced in this regard; however, the best approach is yet open to debate. This study aimed to evaluate the results of complete EXCISION and marsupialization.Materials and Methods: In this retrospective setting, profiles of 320 patients with pilonidal disease operated by EXCISION and marsupialization technique in Tabriz hospitals between 2001 and 2009 were studied. Patients’ demographics, as well as short-term and long-term outcomes were recorded. Results: In total, 320 cases with pilonidal disease, 288 males and 32 females with the mean age of 26.3±8.5 years were enrolled. Majority of the cases (98.8%) were discharged on second day of operation. The mean time of SURGICAL wound healing was 4.4±3.2 weeks after the operation. The one-year recurrence rate was 1.3% and reoperation was indicated in 0.3%. There were a few morbidities without mortality. Conclusion: Complete SURGICAL EXCISION and marsupialization is a safe and effective procedure with a few complications in treating the pilonidal disease.

Abstract:

Case Description: An eight-year-old, light brown and crossbred stallion in a horse-racing club had two gray skin masses on his left thigh and thoracic inlet with a slow growth since two years ago. The masses were 5×7 cm in size, irregular shaped with some convex parts. Surface bleeding had been reported occasionally. CBC and WBC were within normal values. General clinical examination of the animal revealed no problem.Treatment and Outcome: Under general anesthesia (premedication, induction and maintenance by xylazine and ketamine) two neoplastic masses were excised completely. The lesions were restricted to skin with no subcutaneous tissue involvement. Histopathologic results confirmed cutaneous fibroma based on numerous fibroblasts and collagen fibers. There were no recurrences or any other complications in post-operative monitoring for six months.Clinical Relevance: Treatment of cutaneous fibroma is complete by SURGICAL EXCISION in ablating the tumor masses and preventing re-growth.

Abstract:

Introduction: Atrial myxomas are rare benign tumors, causing obstructive or embolic complications, or even death, depending on their site and size. Therefore, once diagnosed, it should be SURGICALly resected. Atrial myxomas are present about 75% in left atrium (LA) and about 15% in right atrium (RA). Early diagnosis is a challenge because of nonspecific manifestations, and sometimes is asymptomatic and may be discovered accidentally during transthoracic echography (TTE). Minimally invasive cardiac surgery (MICS) has benefits over sternotomy include cosmetically, less pain, and shorter total hospital stay. Materials and Methods: Between January 2011 to December 2020, (50) patients [30 Sternotomy, 20 MICS] underwent surgery for isolated resection of cardiac myxoma. We reported outcomes, cardiopulmonary bypass (CPB) time, aortic cross-clamp (ACC) time, conversion to median sternotomy (ST), total hospital stay, complications (stroke, renal failure, respiratory failure, reoperation, and infection), pain, patient, s satisfaction, recurrence and survival. Follow-up time was from 6-months to 3-years. Results: There is no significant difference in CPB or cross-clamp time between groups. No minimal invasive (MI) cases required conversion to a median ST. Total hospital stay is shorter in the MI group by 2. 2 days (p-value = 0. 045). No differences present in morbidity or mortality between two groups. Conclusions: SURGICAL resection of atrial myxoma resection by minimal invasive approach is safe, feasible, and favored over sternotomy.
